Website information and tracking

Updated October 26, 2026. This section describes our website’s collection and use of information and updates the website-tracking disclosures below. It does not change our text-message consent commitments or any separate notice governing health records.

When you send an inquiry, we receive the contact details and message you provide. Please limit your message to what is needed for us to respond. Website servers and embedded services also receive technical information such as your IP address, browser and device information, page URL, referring page, and time of access. Cookies and similar identifiers can distinguish a browser and associate visits and interactions; this information is not necessarily anonymous.

• Google Analytics: website activity and measurement, including pages viewed, navigation, device/browser information, cookie or client identifiers, and configured interaction or form events. A form event can describe the form and submission without containing the message entered in it. Google receives connection information when its services are requested.
• Google Tag Manager: manages the loading of measurement and other website tags; the tags it loads determine the information sent to each provider.
• CallRail: associates website visits and referral or campaign information with inquiries and dynamically displayed telephone numbers. The website’s form-tracking integration also sends submitted contact and admissions form fields to CallRail. These include name, email address, telephone number, message text, date of birth, insurance details and member/group numbers, prior treatment, and additional notes when provided in the admissions form.
• Leadferno: an embedded messaging tool that processes the contact details and message you provide when you use it.
• Google reCAPTCHA: device, browser, and interaction information used for spam and abuse prevention.
• Google Maps: embedded maps receive connection and device information when loaded; interactions with the map are processed by Google.

We use website information to respond to requests, operate and protect the website, understand how visitors find and use it, and measure the effectiveness of our communications. Information is processed by the hosting, communications, measurement, and other providers described here as needed for those functions. Embedded services also have their own privacy terms.

Health-related inquiries

A general question is not automatically protected health information. However, information that identifies someone and concerns their health, requested care, appointments, treatment, or payment for care can be sensitive health information and may be protected by HIPAA or other laws. That can include information submitted before a person becomes a patient. Please do not send medical records, insurance identifiers, Social Security numbers, or detailed health histories through an ordinary website message. Contact our office for the appropriate way to provide sensitive information. This policy and ordinary cookie choices do not replace any authorization required for a use or disclosure of health information.

Privacy choices and requests

Advertising-related disclosures can fall within legal definitions of “sale” or “sharing” even where no money is exchanged. We may need information to verify and respond to a request; legal exceptions can apply.

Retention and changes

The period information is needed depends on its purpose: responding to an inquiry, providing a requested service, maintaining required business or clinical records, respecting communication choices, security, or resolving a dispute. Provider-held website data is also subject to the applicable service settings and terms. You may contact us about retention or deletion of information relating to you.
